A dermatologist is the ideal choice — this is squarely their specialty and their chart notes carry the most weight with insurers. Your primary care provider, a rheumatologist (for psoriatic disease), or a nurse practitioner / physician assistant who can prescribe in your state all work too. The requirement is simple: they must be able to prescribe, have an NPI number, and have chart notes on your condition.

- A statement of medical necessity — the form includes a body surface area calculator.
- Previous treatments you’ve tried and why they weren’t enough — this is what insurers scrutinize most.
- Supporting chart notes, uploaded to the form or faxed separately.
